The Neighborhood Clinic (NC) Social Work Program Coordinator (SWPC) is a full-time LICSW position. The SWPC is responsible for developing, overseeing, and carrying out all aspects of the social work and mental health program under the direction of the Executive Director.
THE MENTAL HEALTH PROGRAM:
The SWPC will develop and coordinate the mental health program. This will include recruitment of volunteer providers and coordinating their time, patients (as necessary) and clinical space at the NC. The SWPC will also develop the outreach and advertisement of the NC mental health program.
The SWPC will be present at all clinic nights (Mondays and Thursdays 5 pm-8 pm) and will see patients for social work needs, schedule follow up appointments with themselves or with the mental health team as appropriate.
The SWPC will triage any urgent social work needs during clinic hours but can schedule follow up for non-urgent issues for a non-clinic day as appropriate. Issues include, but are not limited to housing, Access to care referrals, addiction treatment, mental health treatments/referrals, financial resources (DSHS, SSI, VA, etc.) and assistance with application (can be done during non-clinical hours if needed) immigration issues/referrals.
The SWPC will develop and maintain an up-to-date master community resource list/file for NC patients.
The SWPC creates social work-related intakes and assessment templates and other NC chart or EHR documentation as appropriate. The SWPC also completes biopsychosocial assessments for patients as appropriate and documents assessments and other notes in the patient chart.
The SWPC will collaborate with UWT, PLU, or other CSWE-accredited universities to take MSW
practicum student(s) for clinical practice. The SWPC will supervise and evaluate all MSW practicum students in accordance with university protocols.
The SWPC will create, coordinate, and facilitate mental health and psycho-educational groups in accordance with the patient needs of the NC during non-clinic hours.
Community outreach and communication, to include event planning where appropriate, will be a component of the SWPC job. This position will also be partially responsible for the outreach and education to the community the NC serves including pertinent coalitions and collaborative community groups.
The SWPC will be partially responsible for grant writing and funding this position.
